By attending CCIH events, you consent to having your image and voice captured by CCIH photographers and videographers and on audio recording devices. The resulting materials, including still photographs, video, and audio recordings may be used, without restriction, by CCIH in articles, promotional materials, and on the CCIH website and social media sites. Public Health Protection: CCIH shall monitor public health recommendations from local authorities and may enforce rules such as required masking and social distancing as it becomes necessary.
Medical Release: I release CCIH from any responsibility for medical care, and I understand that participants are solely responsible for all facets of health care that they require. CCIH assumes no responsibility for ensuring access to health care services. I understand that in the United States, uninsured health care expenses can be very high and therefore those without effective health insurance in the United States should obtain a travelers policy for the duration of their stay.
Liability Release: Even with the best of planning and precaution, unforeseen events can occur. By signing this form, I agree to assume and accept all risks and hazards inherent in these activities. I also agree to not hold the event holder (CCIH), or its employees, board or volunteers liable for damages, losses or injuries to the person or property for the undersigned.
